Can someone please assist me with the html necessary to add the image as part of my forum name please.

Ukulele Players Hub is the name.

If you are using the default theme, the easiest (and update safest) way would be to edit that image to include your forum name, upload it to your forum root, and then in Admin -> Configuration -> Current Theme set the "Logo image URL" to point to it. This is how we have the large Simple Machines® Forum banner set at the top as well.

If you are not using the default theme, or would like something more specific, then please share the theme you are using and describe what you want in more detail.

That typically means you are not using the correct url for the logo.
